The content does not diagnose or provide suggested treatments and is not intended to be a substitute for professional medical advice.
• No information or product mentioned on this site is intended to diagnose, treat, cure, or prevent any disease. Not all statements have been evaluated by the Food and Drug Administration. Obligations and Acknowledgements of the User

• The user agrees and acknowledges that Company does not endorse any products offered for sale and that he or she is free to purchase such products from any source. The user agrees to only order the products or utilize the information that he or she has been instructed to do so by his or her licensed health care practitioner. As such, the user releases Company from any liability associated with any of the products, information, or content on this web site whether added by the clinician or maintained by Company.
• There may be circumstances where there are links that allow access to other web sites. Neither the Company, nor its licensors, nor its suppliers, make any representations or give any warranties with respect to any information contained in these other sites and they shall not be liable for any damages or injury arising from the content of these other sites. Neither the Company, nor its licensors, nor it suppliers endorse the individuals, companies, or other similar entities, or any products or materials associated with such individuals, companies or other similar entities.
• The user authorizes the Company to collect personal information (as initiated by the user) and gives his or her consent to the Company to make such personal information available to the responsible health care clinician of this site. The consent form authorizes the disclosure of information relating to the patient to the clinician and representatives of the Company who have agreed to maintain the confidentiality of such information.
• The user agrees to release the Company and its affiliated companies from any liability involving any unauthorized access (stolen passwords, etc.) to this web site or its content including patient profile information. The user shall indemnify the Company and the clinician against any liability or costs incurred by the Company or the clinician in connection with any claim by a third party arising out of the userís receipt of services or any breach of the representations and warranties by the user under this agreement.
• The user represents and warranties that he or she is accessing the web site of his or her health care clinician only and that he or she has contacted said clinician regarding his or her rights as the patient of the responsible health care clinician on this site and the userís ability to utilize the services of this site as instructed to do so by his or her health care clinician.
• The Company may, at its option, immediately terminate this agreement if the user is in default of any of the userís representations, warranties or obligations. Upon termination or expiration of this agreementthe Company shall have the right to disallow access to this site.

Confidentiality

• In addition to the confidentiality obligations set out below, unless required by law the company shall not provide, or make available, the personal medical information of any individual user of the site to any person other than the responsible health care clinician of this site and necessary representatives of the Company. The company will not disclose to unaffiliated companies any information, including e-mail addresses, it gathers from the user other than for the provision of services as contemplated by this agreement.

General

• This agreement supersedes and cancels any and all prior understandings or agreements verbal or otherwise, in relation to such matters which may exist between the parties.  No oral explanation or no oral information given by the parties shall alter the meaning or interpretation of this agreement.  The limitation of liability and confidentiality section of the agreement shall survive the termination or expiration of this agreement.
